The following guidelines were identified during the Evaluator workshop held at JMU on July 21, 2004. |

| Guidelines for Submitters (Teachers) | |||

Be professional in your presentation – this portfolio is a professional document. | ||
| ■ | Need to address each performance indicator for each rubric, as well as the smaller elements within each performance indicator (such as regularly, over time, uses to modify instruction, etc.). | ||
| ■ | Artifacts must be current (within past 2 years), although additional artifacts from past may be included as supplements. | ||
| ■ | If rubric asks for skill to be demonstrated over time, include rubrics that show sequence. | ||
| ■ | Each submission should include two parts: a complete reflection and one or more work-product artifacts that serve as evidence. | ||
| ■ | Recommendations for a complete reflection: | ||
| □ | Explicitly explain how/why artifacts meet the rubric. You may want to use the same phrases as are used in the rubric. | ||
| □ | Each performance indicator should be addressed in the reflection. | ||
| □ | Explain how the artifacts are used to influence your classroom instruction. | ||
| □ | If you mention a work-product in the reflection, submit that item as an artifact in order to provide evidence for your claims. | ||
| □ | You may find it helpful to create your reflection as a Word document and upload it rather than typing your reflection in the box provided. This prevents loss of data in the event of a system time-out and allows you to easily edit the reflection later if a resubmission is required. | ||

Student names cannot be mentioned in reflections and may not appear in artifacts, including student work products and gradebooks, test scores, etc. If you want to submit an artifact that includes student names, the names must be removed before being submitted. Any items that contain student names or other student identifiable information will not be considered for evaluation and will be immediately removed from the system. |
