Parent Field Testing
If you would like to participate in
the field testing of the Learning Toolbox, you
will have to complete the Parent Informed Consent
Form. Your child will also have to participate
so you will have to complete a Parent Informed
Consent Form (for Child) if your child is under
age of 18. If your child is over 18, your child
will have to complete a Student Informed Consent
Form.
Read the Parent Informed Consent Form carefully,
sign it, and mail it to the address on the form.
After it is received you will be notified. At
that time you can participate in the field testing.
If your child is over 18, you can participate
in the field testing after both the Student Informed
Consent Form and the Parent Informed Consent Form
have been received.
