Student Evaluation
If you are under the age of 18 and your
parent has signed and submitted a Parent Informed Consent Form and you have
been notified that the form has been received, proceed to the Student Evaluation
Form.
If you are over the age of 18 and you have signed and submitted a Student
Informed Consent form and you have been notified that the form has been received,
proceed to the Student Evaluation Form.
Do not complete the Evaluation Form until you have become familiar with
it and have used it over time.
- Click on the Student Evaluation Form button below.
- Complete the top section of the
form with your name and e-mail.
- Be sure to answer all of the
questions on the form.
- If you would like to point out
the strengths and weaknesses of the website or make recommendations, type
these in the boxes at the end.
- If you would like to go back
and change any of your responses, use the back button.
- At the end of the form, press
the submit button and your form will be automatically sent.
