Teacher Field Testing

If you would like to participate in the field testing of the Learning Toolbox, you will have to complete the Teacher Informed Consent Form. You can participate in the field testing on your own or with your students. If your students are to be involved in the field testing, you MUST obtain parental permission for each of them. Go to the Student Field Testing page and have the students read the directions for having their parents complete the Parent Informed Consent Form (for Child).

Read the Teacher Informed Consent Form carefully, sign it, and mail it to the address on the form. After it is received you will be notified. At that time you can participate in the field testing.
