Field Testing
If you would like to participate in the
field testing of the Learning Toolbox and you are
under the age of 18, you will have to have
a parent give you permission for you to participate.
Have your parent click on the Parent Informed Consent
Form (for Child), read it carefully, sign it, and
mail it to the address on the form. After it is
received you will be notified. At that time, you
can participate in the field testing.
If you would like to participate in the
field testing of the Learning Toolbox and you are
18 or over, you will have to complete the Student
Informed Consent Form. Read it carefully, sign it,
and mail it to the address on the form. After it
is received you will be notified. At that time,
you can participate in field testing.
